Liability Waiver and Release of Claims for Participants at The Virtuoso Intensive

This Liability Waiver and Release of Claims (the "Agreement") is entered into by the undersigned parent or legal guardian ("Guardian") of the minor child ("Participant") whose name is listed below, in consideration for the Participant’s enrollment and participation in The Virtuoso Intensive, a dance program ("The Program").

1: Acknowledgement of Risks:
The Guardian understands that participation in dance activities carries certain inherent risks, including but not limited to physical injury, falls, muscle strain, joint injuries, or accidents, which may occur during classes, rehearsals, or other Program-related activities. The Guardian acknowledges that such risks exist and voluntarily agrees to allow the Participant to engage in all activities related to The Program.

2: Personal Injury:
The Guardian recognizes and understands that there are potential risks associated with physical activity and voluntarily assumes all responsibility for any injury, loss, or damage resulting from the Participant’s involvement in The Program. The Guardian agrees to indemnify and hold harmless The Virtuoso Intensive, its staff, instructors, volunteers, and affiliates from any and all claims, liabilities, or expenses arising from any injury or harm suffered by the Participant while attending or participating in The Program.

3: Protection of Personal Property:
The Guardian understands that The Virtuoso Intensive does not assume responsibility for the protection, security, or loss of personal property. The Guardian agrees that the Participant is solely responsible for their personal belongings, and The Virtuoso Intensive shall not be liable for any lost, stolen, or damaged property.

4: Medical Attention:
In the event of an emergency, the Guardian grants permission to The Virtuoso Intensive staff and/or medical professionals to administer or seek medical treatment for the Participant, if necessary. The Guardian agrees to be responsible for any medical costs, including but not limited to doctor’s fees, hospital charges, or other related medical expenses incurred as a result of the Participant’s involvement in The Program.

5: Photo and Video Release:
The Guardian grants permission to The Virtuoso Intensive to take and use photographs, videos, and other media (the "Media") of the Participant during The Program for promotional, educational, and marketing purposes, including but not limited to website content, social media, printed materials, and other advertising platforms. The Guardian acknowledges and agrees that such Media may be publicly shared, and that they will not receive any compensation or royalties for the use of the Media.

6: Mailing List Agreement:
By signing up for The Program, the Guardian agrees to join The Virtuoso Intensive mailing list. The Guardian understands that this includes receiving emails about future events, updates, promotions, and other information related to The Virtuoso Intensive. The Guardian acknowledges that they can request to be removed from the mailing list at any time by contacting The Virtuoso Intensive directly.

7: Acknowledgment of Waiver:
By signing below, the Guardian acknowledges that they have read and fully understand the contents of this Agreement, including the risks associated with the Participant’s involvement in The Program. The Guardian voluntarily agrees to release and hold harmless The Virtuoso Intensive, its staff, instructors, volunteers, and affiliates from any and all liability, claims, demands, or causes of action for injury, loss, or damage to the Participant’s person or property. The Guardian further acknowledges that this waiver is binding upon them and their heirs, representatives, and assigns.
